Potential Donor
Your involvement can pave the way for potential donors and raise funds essential for Tracey’s treatment.
The first step is to fill out an online Health History questionnaire. Complete the form when you have adequate time and space to concentrate on each question. Before you begin, please obtain your height and weight.
Using a laptop or desktop computer, visit mayoclinic.org/livingdonor to get started.
Once you have completed the questionnaire, your information will be reviewed by a Living Donor RN Care Coordinator, and you will be contacted by email or phone.
